A: 19 d since graft was performed - site feels well adhered overall, should act as a scab even if all the tissue is not viable. Looks better today than previously described, not sure if there is active infection or if there was just a seroma/fluid build up that needed to drain.
P: Catch EOD, monitor graft site when in hand. Try off Clavamox and pentoxifylline, if wound worsens or shows more signs of infection will have to consider other antibiotic options or further debridement.

O: Several of skin graft margins had dried, yellow/brown scabbing/build up over them. Debrided what would easily come off with tweezers and pearlescent fluid started leaking out of medial edge. Expelled more fluid by pushing on graft area. Cleaned entire area with chlorahex and applied TAO to margins. Gave meds PO. A: Some signs of infection at skin graft area in the form of pearlescent fluid. Given how sealed the edges of the graft were, may have been building up for some time. Estimate 0.20cc fluid expressed. Some of build up at margins of graft may be dried exudate that was leaking through. Graft starting to look more scab like in spots, but several areas still appear to be relatively healthy tissue.
P: Vet check of graft sight tomorrow. Start cleaning margins of graft SID and expressing any fluid present.

A: Removed non-viable skin and muscle over the cranial aspect of the L leg. Very large defect left after debridement, had to place a skin graft to cover wound site - very large, high tension area, at risk for dehiscence and graft failure. Not sure how well leg will function after deep muscle debridement as well - very guarded prognosis.
P: Change bandage daily to monitor graft site. Monitor donor graft site for dehiscence. Monitor use of L leg.

Plucked and scrubbed area wound on L TBT and R inguinal area
for surgery. Debrided white/non-viable skin and outer layer
of muscle until got to bleeding, viable tissue. Flushed area
with saline. Gave 30 cc saline SQ over the R inguinal, removed
2x4 sized area of skin from this area and placed in saline.
Partially closed donor graft site with 4-0 webcryl simple interrupted
sutures (enough that the underlying SQ tissue was covered and
would not dry out). Tacked the edges of the skin graft site
to the edges of the wound with 4-0 webcryl. Using simple interrupted
and horizontal mattress sutures with 4-0 webcryl, sutured graft
in place and closed excess skin on edges. Left 2 meshes on the
graft site. Finished closure of the donor graft site with additional
interrupted sutures. Covered graft recipient site with SSD,
telfa/tegaderm and papertape. Left donor site uncovered. Patient
was slow to recover from anesthesia. |
